Not For Human Consumption (Yet)

Retatrutide has never been approved by any regulator anywhere. Eli Lilly does not expect to file for FDA approval until Q1 2027, and cardiovascular outcome data will not exist until 2029.

It is also, right now, the most heavily lab-tested peptide in America. One independent testing service has published 3,619 assays on retatrutide samples submitted by consumers, sourced from 250(!) different vendors. That is more tests than tirzepatide, and almost 10x more than semaglutide. So if it is not approved by any regulators, but it has 250 different vendors, what is going on here? The market is just massive.

Parabolic Adoption

The adoption we’ve seen in the peptide space is one of the fastest in the history of pharmacology. Statins needed roughly a decade to reach comparable numbers. GLP-1s did that in three years, mostly relying on cash payment. By June 2026, more than 8 of every 100 prescriptions written in the US were GLP-1 receptor agonists. Medicare Part D GLP-1 claims went from 4.8M in 2019 to 21.8M in 2024, a span in which gross spend jumped from $5.5B to $27.5B.

Studies estimate that 11% of US adults are currently on a GLP-1 , and 15% have used one before. Benefitting from this, Eli Lilly has seen a Q2 2026 revenue of $23.0B, which is up 47% YoY. Mounjaro alone brought $9.9B worldwide in the quarter, and the estimate for the full fiscal year is estimated to be between $85-87B. However, despite Eli Lilly’s position on retatrutide and their legal ownerships, they are not providing the peptide to consumers/patients yet. With that being said, there are seemingly hundreds of thousands of people online raving about its benefits. Where are they sourcing this from?

The answer is a tremendous grey market.

The Grey Market, Mechanically

Between March 2023 and September 2024, customs data showed that 268kg of bulk semaglutide entered the US markets across 159 shipments from 16 companies. If we take the starter dose to be 0.25mg, this is nearly ~4000 starter doses per gram, which comes out to nearly a billion starter doses. Chainalysis and TRM labs traced peptide revenue flowing to Chinese firms that previously were selling fentanyl precursors, reusing the same shipping and cryptocurrency rails. Shanghai Sigma Audley, which was doing over $1M a year in Bitcoin flows from precursors, pivoted to peptides in early 2025. Shocking!

These crypto inflows to grey market vendors grew from $200K a month pre-2025 to $32M in Q1 2026, with $39M projected for Q2 (implying a run rate over $100M). It’s worth noting that this is CRYPTO ONLY, so we are not looking at cards, Zelle, CashApp, wire, or cash transactions. With such a massive surge in sales for grey market peptides, it’s only natural to wonder how people are getting drawn to this market.
